Including summer school, for your child to receive a vaccine or any health services at school-based health clinics or from the school nurse, HCS must have a completed consent packet. Forms are sent home with children at the beginning of each school term for all health services and before any vaccine program. Please complete and sign all forms and return them to your school as soon as possible after you receive them. If you have not received the necessary forms and need more information, please contact your school's main office.
